Who Megumi is
Megumi is the quiet one of Gojo's three first-years, the son Toji Fushiguro sold to the Zenin clan before Gojo bought the deal out. He carries the Ten Shadows Technique, the Zenin inheritance that summons shikigami from shadow: the Divine Dogs, Nue, the Great Serpent, the toads, and the one nobody has ever tamed, Mahoraga.
He fights carefully and hates it, because he knows he is holding back. Sukuna notices him in chapter 9 and never looks away. His stepsister Tsumiki, cursed and in a coma, is why he stays.
What happens to Megumi
Manga spoilers, through the last chapter.
In the Culling Game, Tsumiki turns out to be possessed by the ancient sorcerer Yorozu. Sukuna leaves Yuji, takes Megumi's body in chapter 212, and kills Tsumiki in front of what is left of him. Megumi spends the rest of the manga drowning inside his own body while Sukuna uses the Ten Shadows and tames Mahoraga. Yuji and the others pull him back in the last fight. He is alive at the end, and the shikigami are gone.

- No. Sukuna takes his body in chapter 212 and wears it through the Shinjuku showdown, but Megumi is pulled back and is alive at the end of the manga.
